Job summary

Mental Health America of Los Angeles (MHALA) is seeking a Veteran Healthcare Navigator to support healthcare access, housing stability, and care coordination for Veterans enrolled in the SSVF program. This role collaborates with VA partners to reduce barriers to care and housing.

Under supervision, you will complete HMIS documentation, assist with enrollment in VA healthcare and public benefits, and promote continuity of care using trauma-informed, client-centered practices.

Qualifications

• Bachelor’s degree in human services, preferred.
• One year of providing psychiatric rehabilitation services to a disabled or unhoused population preferred.
• Knowledge of healthcare and social service resource systems.

Responsibilities

• Provide veteran-centered healthcare access and housing coordination.
• Maintain accurate case files and HMIS documentation.
• Coordinate referrals and warm hand-offs to VA and community providers.
• Assist with enrollment in VA healthcare and public benefits.
• Engage in staff meetings and support program operations.
