Job summary

Western Reserve Area Agency on Aging in North Olmsted, OH seeks a Full Time Veteran-Directed Care Manager to coordinate referrals, conduct in‑person intake assessments, and educate Veterans about the VDC program. You will travel to Veterans’ homes or agreed venues and document outcomes.

You will help create and annually review Spending Plans, ensure compliance with policy and procedures, report to supervisors, and participate in VA training and cross‑training across programs.

Qualifications

• Education and/or Experience: bachelor’s degree or equivalent combination.
• Veteran Preferred.
• Exceptional interpersonal and communication skills.
• Ability to travel within service area (Lake, Lorain, Medina, Cuyahoga, Geauga).
• Proficiency with Microsoft Office suite and related software.

Responsibilities

• Respond to referrals for Veteran Directed Care and schedule intake meetings.
• Conduct face-to-face intake sessions within 5 days of intake.
• Document all interactions in ECR within 24 hours.
• Educate Veterans about the VDC program and develop Spending Plans.
• Collaborate with Veterans to budget goods/services per policy.
• Present Spending Plans for supervisor review and adjust as needed.
• Maintain monthly communication with Veterans and quarterly face-to-face meetings.
• Cross-train in additional agency programs as required.
