Job summary

ElderSource in Northeast Florida is seeking a VDC Person-Centered Counselor to coordinate the Veteran-Directed Care program with ElderSource, the VA, veteran beneficiaries, and fiscal staff. Under the supervision of the Direct Programs Manager, you provide technical assistance and act as a consultant to veterans and their representatives.

The role requires frequent travel across Northeast Florida, strong data handling, budget awareness, and on-going communication with funders and program

Qualifications

• Bachelor’s degree or four years related experience.
• Ability to read, analyze, and interpret general business materials.
• Ability to write reports, business correspondence, and procedures.
• Ability to effectively present information to groups.

Responsibilities

• Provide program orientation and training for veterans.
• Conduct home-based visits and develop service plans.
• Coordinate invoices and budgeting with fiscal staff.
• Ensure compliance with legal and regulatory requirements.
• Maintain confidentiality of client data.
• Attend conferences and disaster-related duties as assigned.
