Job summary

Coastal Harbor in Georgia seeks a Case Manager to coordinate admissions and after-care for service members, veterans, or dependents. This role follows individualized treatment plans and links patients to community and military resources to support independence.

You will work with the Director of Military Programs to ensure timely communication with bases and referral sources, complete documentation, and advocate within a multidisciplinary team.

Qualifications

• Bachelor’s degree in Human Services with at least two years of healthcare experience.
• Ability to focus on lead generation, quality referral source development, as well as maintenance of current accounts.
• Extensive knowledge of military culture.
• Ability to perform assignments with minimal supervision.
• Sound, independent judgments based on ethical principles.
• Strong reading, written and verbal communication skills.
• Valid state driver’s license with appropriate endorsement.

Responsibilities

• Coordinate admissions and after-care needs based on clinicians' recommendations and program goals.
• Familiarize with community and military resources and make referrals to access support networks.
• Ensure all documents are sent to the appropriate team or agency and record follow-up appointments in discharge instructions.
• Collaborate with clinicians, service members, and community/military agencies to link patients to continuing care resources.
• Advocate for the patient as part of a multidisciplinary treatment team.
