The incumbent serves as one of several Care Coordinators in the Office of Community Care (OCC) at the Roseburg VA Health Care System (RVAHCS). Care Coordinators administratively processes care in a variety of areas within OCC.

They coordinate care for Veterans with non-VA providers, review requests, ensure compliance with policies, schedule timely services, and document actions in the medical record with a professional, customer-focused approach.

Aufgaben

• Coordinate care for Veterans with non-VA providers and ensure timely scheduling and delivery of care.
• Communicate with Veterans, vendors, family, and staff in a professional, friendly and helpful manner.
• Document actions, communications, and follow-up in the medical record with accuracy and professionalism.
• Respond to Veteran secure messages and department protocols in a timely manner.
• Work Schedule: Monday - Friday 8:00am - 4:30pm; Telework: Not Available; This is not a virtual position.
