Job summary

The Registered Nurse (RN) Community Care (CC) Coordinator at Veterans Health Administration, Department of Veterans Affairs coordinates care for Veterans across VA and community providers, triages referrals, and ensures timely access to services.

You will apply the nursing process, collaborate with interdisciplinary teams, and guide Veterans through consults and care options while adhering to VA guidelines and quality standards.

Qualifications

• Must be a graduate of an approved nursing program and licensed in the United States.
• Experience with care coordination and community care processes is preferred.
• Knowledge of VA Community Care programs and VA clinical systems is desirable.

Responsibilities

• Coordinate and triage referrals for Veterans receiving care in the community.
• Collaborate with VA and community providers to ensure timely access to care.
• Support and guide Veterans through the consult process and care options.
