Job summary

Southend Pharmacy in Houston, TX is seeking an experienced Verification Pharmacist to join our fast-paced compounding operation. You will review prescriptions, verify compounded medications, and ensure compliance with USP standards and state regulations to safeguard patient safety.

This full-time, onsite role requires strong clinical judgment, meticulous attention to detail, and the ability to collaborate with pharmacy technicians and leadership to maintain high-quality standards and efficient

Qualifications

• Doctor of Pharmacy (Pharm.D.) or B.S. in Pharmacy from ACPE-accredited school.
• Active, unrestricted Texas Pharmacist License in good standing with the Texas State Board of Pharmacy (TSBP).
• Minimum of four years licensed pharmacist experience in a high-volume environment.
• Strong understanding of pharmacy operations, medication verification, prescription review, and patient safety practices.
• Working knowledge of USP standards, Texas State Board regulations, DEA requirements, and HIPAA.

Responsibilities

• Perform final pharmacist verification of compounded prescriptions prior to dispensing, ensuring accuracy and compliance.
• Verify prescription orders for clinical appropriateness, dosage accuracy, patient information, drug interactions, and labeling requirements.
• Review compounded medications to ensure adherence to USP standards, Texas regulations, DEA requirements, and SOPs.
• Ensure all documentation, Master Formulation Records, batch records, and quality documentation are complete and accurate.
• Collaborate closely with sterile and non-sterile technicians to maintain efficient workflow while upholding quality.
• Support verification activities for high-volume pharmacy operations while maintaining accuracy and patient safety.
• Assist in identifying and resolving prescription discrepancies, formulation questions, documentation issues, and medication-related concerns.
• Provide clinical guidance and mentorship to pharmacy technicians regarding prescription processing and regulatory requirements.
• Maintain compliance with pharmacy policies, QA initiatives, and regulatory inspection readiness.
• Participate in continuous quality improvement and workflow optimization projects.
• Monitor medication inventory and minimize waste while ensuring availability.
• Maintain PHI confidentiality per HIPAA.
