Qualifications

• BLS certification required.

• Experience in nursing practice and patient-centered care is preferred.

Responsibilities

• Performs a comprehensive patient assessment with documented results.
• Communicates changes in patient condition promptly.
• Uses nursing diagnosis to set measurable goals and updates the care plan.
• Collaborates with patient, family, and health-care team for care planning and discharge planning.
• Evaluates patient responses to interventions and adjusts the care plan as needed.
• Ensures discharge instructions and medication reconciliation are completed and explained to patient/family.
