Job summary

The RN will perform comprehensive assessments, coordinate care with families and the health care team, manage discharge planning, and ensure accurate medication reconciliation.

Qualifications

• BSN required and licensed in Illinois.
• Current State of Illinois Registered Professional Nurse.
• Basic Life Support certification.
• ADN may be acceptable with commitment to obtain BSN within three years of first day of employment in clinical nurse role.

Responsibilities

• Performs a comprehensive assessment on all assigned patients with documented results.
• Communicates changes in patient’s condition promptly.
• Derives realistic outcomes and documents measurable goals on the plan of care; updates it as needed.
• Collaborates with patient, family, and health care team for care planning and documents accordingly.
• Assesses discharge planning needs and makes appropriate referrals.
• Evaluates patient responses to nursing interventions and adjusts the plan of care.
• Ensures discharge instructions and medication reconciliation are completed and patient education is provided.
