• Process and coordinate vascular referrals for patients.
• Send referrals to vascular specialists, imaging centers and other providers.
• Contact outside offices to coordinate appointments and obtain referral information.
• Track outstanding referrals and follow up to ensure completion.
• Communicate referral status with patients as needed.
• Document referrals accurately in the electronic medical record.
• Follow up on referrals not yet scheduled or completed.
• Maintain referral tracking and documentation.
• Process medical records requests from patients, attorneys, insurers and providers.
• Review requests for proper authorization and documentation.
• Gather and prepare requested records for release.
• Respond to requests promptly and accurately.
• Notify relevant parties about incomplete requests or delays.
• Ensure records are released only to authorized individuals or organizations.
• Maintain documentation of all requests and releases.
• Adhere to HIPAA, privacy regulations and company policies.
