Job summary

The Hillingdon Hospitals NHS Foundation Trust is seeking a Vascular Patient Pathway Coordinator to lead the tracking of outpatient RTT pathways. You will work closely with clinical and managerial staff to ensure patients receive timely treatment and national targets are met, providing high-standard administrative support across Trust-wide specialities.

Qualifications

• Experience coordinating patient pathways and tracking RTT pathways.
• Ability to work with clinicians, managers, GPs and other organisations to resolve delays.
• Strong administrative skills and data/record-keeping abilities.
• Experience analysing pathway data and supporting audits.

Responsibilities

• Monitor and coordinate patients throughout their care pathway using tracking lists to identify next steps.
• Manage referrals and pathway administration in line with RTT rules.
• Work with clinicians and managers to resolve pathway delays and escalate breaches.
• Maintain accurate patient records and databases, track health records securely, and support clinical correspondence.
• Coordinate information and paperwork for clinical teams and meetings; follow up actions.
