Job summary

Apex Vascular is seeking an Advanced Practice Provider (NP/PA) to join our vascular care team at Tennova North, with weekly rotation to Oak Ridge. The APP will deliver comprehensive, patient-centered care and collaborate closely with physicians, nurses, and staff to support vascular health outcomes.

Based primarily at Tennova North, the role requires assessing health needs, developing care plans, coordinating services, and providing patient education on disease management and adherence.

Qualifications

• Graduate of an accredited NP or PA program.
• Current RN license with NP certification, or PA license.
• BLS and ACLS certifications on hand.

Responsibilities

• Conduct comprehensive assessments and develop individualized care plans.
• Coordinate and monitor patient care activities, including referrals.
• Collaborate with physicians, nurses, and staff on care plans.
• Educate patients on disease management and medication adherence.
• Address barriers to care and social determinants of health.
• Document assessments and outcomes in a timely manner.
• Participate in interdisciplinary team meetings.
