Apex Vascular is seeking an Advanced Practice Provider (NP/PA) to join our vascular care team at Tennova North, with weekly rotation to Oak Ridge. Qualifications

• Graduate of an accredited NP or PA program.
• Current RN license with NP certification, or PA license.
• BLS and ACLS certifications on hand.

Responsibilities

Tennova North Office | Monday–Thursday, 8:00 AM–4:30 PM, Friday 8:00 AM–12:00 PM | Weekly rotation to Oak Ridge Office

Apex Vascular is seeking a dedicated and compassionate Advanced Practice Provider (APP) — Nurse Practitioner or Physician Assistant — to join our growing vascular care team. Based primarily at our Tennova North location, this provider will deliver comprehensive, patient-centered care while collaborating closely with physicians, nurses, and interdisciplinary staff to support outstanding vascular health outcomes across our patient community.

About the Role

The APP provides comprehensive support to patients by assessing health needs, developing individualized care plans, and coordinating services across the vascular care continuum. This role ensures continuity and quality of care by collaborating with physicians, nurses, and interdisciplinary team members, facilitating appointments and referrals, and helping patients navigate the healthcare system.

You’ll deliver patient education on disease management and medication adherence, identify and address barriers to care — including social determinants of health — and advocate for patient rights and preferences. You’ll also be responsible for accurate and timely documentation, medication support within established protocols, triage and clinical assessments, and adherence to regulatory and organizational standards.

Schedule & Location

• Primary location: Tennova North office
• Office hours: Monday–Thursday, 8:00 AM – 4:30 PM; Friday, 8:00 AM – 12:00 PM
• Rotates to our Oak Ridge satellite office approximately once per week
• May also cover other satellite clinic locations and hospital rounding as needed

What You'll Do

• Conduct comprehensive assessments of patients' health needs and develop individualized care plans
• Coordinate and monitor patient care activities, including appointments, treatments, and referrals
• Collaborate with physicians, nurses, and other medical staff to plan current and long-term patient care
• Provide patient education on disease management, medication adherence, and lifestyle modifications
• Assess and address barriers to care, including social determinants of health and resource limitations
• Administer care and triage to patients, and order tests, medications, or procedures as needed
• Assist with medication management and administration according to prescribed protocols
• Document patient assessments, interventions, and outcomes accurately and in a timely manner
• Participate in interdisciplinary care team meetings to discuss patient progress and care plans
• Advocate for patients' rights and preferences within the healthcare system
• Maintain compliance with regulatory requirements and professional standards of practice
• Foster a safe, team-oriented, and respectful work environment
• Perform other duties as assigned

What We're Looking For

• Graduate of an accredited Nurse Practitioner or Physician Assistant program
• Current RN license with Nurse Practitioner certification, or current Physician Assistant license
• Current Basic Life Support (BLS) certification
• Current Advanced Cardiovascular Life Support (ACLS) certification
• Valid state driver's license and required minimum auto insurance coverage
• Strong communication, critical thinking, and problem-solving skills
• Working knowledge of Office 365 and EMR systems

Preferred Qualifications

• Two or more years of clinical experience in vascular care
