Job summary

Diversicare seeks a compassionate Certified Nurse Assistant to assist with resident transport and accompany visits, ensuring safety and documentation accuracy. The role supports daily care needs at our Oxford-area facility in Alabama.

Qualifications

• Certified Nurse Assistant (C.N.A.) required.
• Valid state driver’s license required.
• Physically able to assist ambulatory and non-ambulatory residents.

Responsibilities

• Assure the safety of each resident when transporting to and from the doctor’s office and/or hospital.
• Accompany each resident on all medical visits. Stay with resident(s) until visit is complete.
• Observe required sign-in and document handling at visits.
• Obtain and verify resident documents and prescriptions after visits.
• Report to the RCM after returning to the facility and address follow-up questions.

Responsibilities

• Assure the safety of each resident when transporting to and from the doctor’s office and/or hospital.
• Accompany each resident on all medical visits. Stay with resident(s) until visit is complete.
• Observe the following guidelines at each visit: a. Sign in resident upon entrance. b. Present resident’s documents to office personnel.
• Before leaving doctor’s office, please observe the following: a. Obtain resident’s documents. b. Obtain any new prescriptions. c. Check to make sure doctor has written on the doctor’s order form. d. If follow-up appointment necessary, obtain appointment cards.
• After returning to facility, report to RCM and be prepared to answer questions concerning visit.
• Performs other duties as required and assigned.

Qualifications

• Physically able to help ambulatory and non-ambulatory residents transfer to and from wheelchair to vehicle.
• Possess a valid state driver’s license.
• Qualified as a Certified Nurse Assistant (C.N.A.).
• Clean driving record.
• Clean criminal record.
• Any other state required certifications
