Oak Street Health is seeking a Primary Care Physician in Chicago to deliver equitable, value-based care to Medicare populations at our neighborhood centers. You will work with large care teams to emphasize quality outcomes over volume.

Qualifications

• MD or DO required for practice.
• Board certification in Internal Medicine or Family Medicine or eligibility.
• Active, unrestricted State Medical License.
• Active DEA license is required.
• US work authorization is required to work in the US.
• Candidate should embody Oak Street Health values (Oaky).

Responsibilities

• Assess and diagnose patients at local clinics.
• Oversee, direct, and administer primary care.
• Prescribe and administer pharmaceutical treatments as needed.
• Maintain patient electronic medical records (EMR).
• Collaborate with regional and central leadership to meet quality goals.
• Work with Practice Managers to direct the center care team.
• Other duties as assigned.
