Job summary

gobravvo is seeking a board-certified or board-eligible Primary Care Physician (MD/DO) to join a New Haven, CT network of neighborhood centers focused on equitable, value-based care for Medicare populations. The role emphasizes preventive medicine, patient experience, and low-volume, high-quality visits in a collaborative team setting.

You will work with a multidisciplinary team, including scribes and MAs, in centers equipped with onsite labs and access to specialist consultations.

Qualifications

• Must be a board-certified or board-eligible MD/DO in Internal Medicine or Family Medicine.
• Active, unrestricted state medical license.
• Fellowship in geriatrics preferred but not required.

Responsibilities

• Deliver comprehensive primary care to Medicare populations.
• Conduct regular patient visits (time per visit reflects program standards).
• Collaborate with scribes, MAs, social workers and care team members.
• Utilize Canopy and Greenway EMR for documentation and management.
• Order and interpret labs with onsite phlebotomy support.
• Engage in value-based care to reduce hospital admissions and improve outcomes.
• Participate in community health initiatives and telehealth consultations.
