• MD or DO degree and board eligibility/certification in Internal Medicine or Family Medicine
• Active, unrestricted medical license
• Spanish language proficiency preferred

• Fellowship in geriatrics a plus, not required

• Deliver primary care to Medicare patients with a focus on preventive, value-based care
• Collaborate with a multidisciplinary team (scribes, MAs, social workers) to coordinate care
