Job summary

HarmonyCares is seeking physicians to provide in-home primary care for high-acuity geriatric patients, operating Monday through Friday with no weekend or evening obligations.

You will work with a supportive team of providers and staff, leveraging mobile diagnostic services and advanced technology to deliver thorough chronic disease management in patients’ homes.

Qualifications

• Geriatrics or complex patients experience preferred.
• Active/unrestricted medical license and DEA.
• Board Certification or Board Eligible.
• EMR experience.
• Must maintain a valid driver’s license and good driving record.

Overview

Value-based Primary Care Physician role to manage chronic conditions of high-risk geriatric patients Monday-Friday (no weekends or evenings). Spend 30-45 minutes with your patients to manage all chronic conditions alongside a supportive team of providers and office operations staff.
