Job summary

HarmonyCares is seeking physicians to provide in-home primary care for high-acuity geriatric patients in a Monday–Friday schedule. You will work with a supportive team of providers and office staff to manage chronic conditions during patient visits at home.

The role emphasizes compassionate, patient-centered care delivered in the home, with competitive benefits and a physician-driven model. This is a full-time, office-visit workflow with no weekends or evenings.

Qualifications

• Geriatrics or complex patient care experience preferred.
• Active/unrestricted medical license and DEA.
• Board certification or eligibility.
• Experience with electronic medical records (EMR).
• Must have a valid driver’s license and clean driving record.

Responsibilities

• Deliver primary health care and manage patient care in home setting.
• Coordinate care with a multidisciplinary team.
• Develop and implement care plans for high-acuity geriatric patients.

Overview

Value-based Primary Care Physician role to manage chronic conditions of high-risk geriatric patients Monday-Friday (no weekends or evenings). Spend 30-45 minutes with your patients to manage all chronic conditions alongside a supportive team of providers and office operations staff.
