Job summary

HarmonyCares is seeking a Value-based Advance Practice Provider to manage a high-risk geriatric in-home panel, Monday through Friday with no weekends or evenings. Athena EMR and AI Scribe will support charting during the workday.

• Federal DEA License/eligibility
• Geriatric training/experience preferred
• Active CPR/BLS Certification

Responsibilities

The Advance Practice Providers we seek are those who have directly related experience in geriatric primary care managing chronic conditions in alignment to HEDIS/STAR quality outcomes. The Nurse Practitioner who wants to practice medicine with a focus on patient care, not volume. We want our clinicians to take the time needed to truly address the patient’s needs.
