Job summary

HarmonyCares seeks a Value-based Primary Care Physician to manage chronic conditions of high-risk geriatric patients in home settings, Monday through Friday with no weekends or evenings. You will spend 30-45 minutes with each patient, supported by a team of providers and office staff.

Qualifications

• Geriatrics or complex patients experience preferred.
