Job summary

Oak Street Health is seeking a Primary Care Physician to provide equitable and effective value-based healthcare to local Medicare populations at our neighborhood primary care centers. Physicians work within a large care team to focus on quality of care rather than volume.

Qualifications

• MD or DO graduate.
• Board certification in Internal or Family Medicine (or eligible).
• Active, unrestricted State License.
• Active DEA license.
• US work authorization.

Responsibilities

• Assess and diagnose patients at local clinics.
• Oversee, direct, and administer primary care.
• Prescribe and manage medications.
• Maintain Patient EMR data via canopy and greenway.
• Collaborate with leadership to meet health quality goals.
• Work with Practice Managers to direct the center care team.
• Other duties as assigned.
