Job summary

Oak Street Health is seeking a Primary Care Physician in Fort Wayne, IN to deliver value-based care to local Medicare populations. You will work within a neighborhood primary care center supported by large care teams to emphasize outcomes and high-quality care.

Qualifications

• MD or DO medical degree (MD or DO) required.
• US medical license and DEA license required.
• US work authorization required.

Responsibilities

• Assess and diagnose patients at local clinics.
• Oversee, direct, and administer primary care.
• Prescribe and administer pharmaceutical treatments and medications.
• Maintain Patient Electronic Medical Record data via canopy and greenway systems.
• Collaborate with regional and central leadership to meet health quality goals.
• Work with Practice Managers to direct and manage the center care team.
• Other duties as assigned.
