Job summary

Oak Street Health in Albuquerque, NM seeks a Primary Care Physician to deliver value-based, patient-centered care to Medicare populations at our neighborhood centers. You will work with large care teams to improve care quality and reduce hospitalizations.

Qualifications

  • MD or DO graduate with board certification or eligibility.
  • Active, unrestricted State Medical License required.
  • DEA license required.
  • US work authorization required.

Responsibilities

  • Assess and diagnose patients at local clinics.
  • Oversee, direct, and administer primary care.
  • Prescribe and administer pharmaceutical treatments and medications.
  • Maintain Patient Electronic Medical Record data via EMR platforms Canopy and Greenway.
  • Collaborate with regional and central leadership to meet health quality goals.
  • Work with Practice Managers to direct and manage the center care team.
  • Other duties as assigned.
