Oak Street Health, a CVS Health® subsidiary, seeks a Nurse Practitioner to deliver value-based primary care to adults on Medicare in outpatient clinics. You will work within a collaborative care team to manage preventative care, chronic conditions, and care coordination.

The role requires NP certification, applicable state license, DEA license, and US work authorization, with anticipated 32 hours weekly and full-time status. Competitive benefits and pay range are provided.

Qualifications

• Master of Science in Nursing, Doctor of Nursing Practice, or NP Certificate Program graduate.
• National certification in Family NP, Adult-Gerontology, Adult, or Gerontological NP.
• Active, non-probationary NP license
• Active DEA license
• US work authorization

Responsabilités

• Provide exceptional primary care.
• Conduct office visits for routine and acute issues.
• Administer annual wellness visits and HRAs with thorough documentation.
• Coordinate care with other providers and facilities.
• Lead population health efforts with the care team.
• Assist with phone triage and outreach.
• Educate patients on health conditions and care plans.
• Participate in Oak Street Health promotional activities.
• Conduct home visits as needed.
• Perform other duties as assigned.
