Job summary

Oak Street Health is seeking a Nurse Practitioner to provide effective, value-based primary care to adults on Medicare. You will practice in outpatient clinics, collaborate with a care team, and focus on long-term patient relationships.

Qualifications

• Master of Science in Nursing, Doctor of Nursing Practice, or NP certificate program graduate.
• National certification in a primary care NP specialty (Family, AGPCNP, ANP, or GNP).
• Active, non-probationary state NP license.
• Active DEA license.
• US work authorization.

Responsibilities

• Provision of exceptional primary care.
• Conduct office visits for routine and acute issues.
• Administer Annual wellness visits and health risk assessments.
• Care coordination with other providers and facilities.
• Population health leadership with the care team.
• Assisting with phone triage and outreach.
• Educating patients on health conditions and care plans.
• Participating in Oak Street Health promotional activities.
• Conducting home visits as needed.
• Other duties, as assigned.

Education

MSN / DNP or NP certificate program graduate

National NP certification (Family NP, AGPCNP, ANP, GNP)

Active NP license (state)

Active DEA license
