Formación

Responsabilidades

• Provide exceptional primary care to adults on Medicare.
• Conduct office visits for routine and acute issues.
• Administer wellness visits and health risk assessments.
• Coordinate care with providers, facilities and agencies.
• Lead population health initiatives with the care team.
• Educate patients on their health conditions and treatments.
• Participate in Oak Street Health promotional activities.
• Conduct home visits as needed.
• Other duties as assigned.
