Job summary

Oak Street Health in Indianapolis is hiring a Nurse Practitioner to deliver value-based primary care to adults on Medicare. You will work within a collaborative care team to provide longitudinal care and build strong patient relationships.

Roles include conducting wellness visits, coordinating with physicians and specialists, and educating patients about their health plans. This position is full-time with standard weekday hours and a comprehensive benefits package.

Qualifications

• Must have Master of Science in Nursing, Doctor of Nursing Practice, or NP certificate program.
• National certification in one of the following specialties: Family NP, Adult-Gerontology Primary Care NP, Adult NP, or Gerontological NP.
• Active, non-probationary state NP license and active DEA license.
• US work authorization required.

Responsibilities

• Provide effective primary care to adults on Medicare.
• See patients independently and collaborate with physicians.
• Conduct office visits for routine and acute issues and annual wellness visits.
• Coordinate care with specialists, testing facilities, and care managers.
• Educate patients on health conditions and care plans.
• Participate in outreach and promotional activities.
