Qualifications

• Master of Science in Nursing, Doctor of Nursing Practice, or NP Certificate Program graduate.
• Active state NP license in good standing.
• Active DEA license.
• US Work Authorization.
• National certification in at least one NP specialty (Family/Adult-Gerontology/Adult/ Gerontological).

Responsibilities

• Provide exceptional primary care.
• Conduct office visits for routine and acute issues.
• Administer Annual Wellness Visits and health risk assessments with thorough documentation.
• Coordinate care with physicians, specialists, testing facilities and agencies.
• Lead population health initiatives in coordination with the care team.
