Qualifications

• Master of Science in Nursing, Doctor of Nursing Practice, or NP Certificate Program graduate
• National certification in Family/Adult-Gerontology/Primary Care/Adult Nurse Practitioner/Gerontological Nurse Practitioner
• Active, non-probationary state NP license
• Active DEA license; US work authorization

Responsibilities

• Provide exceptional primary care.
• Conduct office visits for routine and acute issues.
• Administer Annual wellness visits and health risk assessments with thorough documentation.
• Coordinate care with other providers, specialists, testing facilities, and agencies.
• Lead population health initiatives with the Care Team, including preventive screenings.
• Assist care team with phone triage and outreach.
• Educate patients on health conditions, care plans, and treatments.
• Participate in Oak Street Health promotional activities.
• Conduct home visits as needed.
• Perform other duties as assigned.
