Job summary

Upperline Health Plus seeks a Primary Care Medical Assistant to deliver high-quality, patient‑centered care in a value‑based practice. You will support rooming, vitals, histories and coordination across the care team in a fast-paced clinic.

Ideal candidates are organized, compassionate, and capable of working at the top of their scope with a collaborative approach. On-site at Columbus, IN with potential cross-location support; travel to Seymour as needed.

Qualifications

• Certified MA with minimum 2 years in a primary care setting.
• Experience in outpatient clinic environments preferred.
• Phlebotomy certification preferred.
• Strong knowledge of medical terminology, disease processes, and pharmacology basics.
• Ability to interpret provider documentation, orders, and treatment plans.
• Care coordination or care management experience ideal.
• Experience caring for adult/geriatric patients preferred.
• Wound care and foot care experience is a plus.
• Bilingual English/Spanish helpful.
• Team-based care collaboration capability.
• Experience with EMR systems (AthenaHealth) desired.
• Proficiency with Word, Excel, and Office tools.
• Reliable transportation and valid license with required auto insurance.
• Excellent customer-service orientation and work ethic.
• Strong multitasking and prioritization in fast-paced settings.
• Empathy, professionalism, and cultural sensitivity.
• Self-starter with problem-solving skills.
• Clear oral and written communication with patients and teams.

Responsibilities

• Room patients, document vitals, history, and chief complaints.
• Assist NP with health assessments during clinic visits.
• Support virtual provider encounters from clinic setting.
• Document clinical data in the EHR.
• Perform medication reconciliations and review meds.
• Identify discrepancies and ensure updated med lists.
• Conduct outreach to coordinate preventive care.
• Handle messages, refills, and prior authorizations.
• Support care coordination tasks and referrals.
• Assist with resource access and community services for patients.
• Prepare equipment and supplies for provider use.
• Collaborate with providers, nurses, and care managers.
• Support workflow optimization and patient flow.
• Identify barriers to care and escalate as needed.
• Participate in team huddles and meetings.
• Perform injections, phlebotomy, and point-of-care testing.
• Other duties as needed by Upperline Health.
