Job summary

Upperline Health Plus is seeking a Primary Care Medical Assistant who delivers high-quality, patient-centered care in a value-based practice.

In our patient-centered and outcomes-driven environment, the Medical Assistant supports patients in the clinic by rooming patients, obtaining medical history, and taking vital signs, while also ensuring care beyond the visit through proactive outreach and care coordination.

Qualifications

• Certified Medical Assistant or minimum of two years’ experience in primary care.
• Strong knowledge of medical terminology and basic pharmacology.
• Experience with team-based care models.

Responsibilities

• Room patients; obtain and document vital signs, medical history, and chief complaints.
• Assist providers with in-clinic visits and health assessments.
• Support virtual provider encounters from the clinic setting.
• Accurately document clinical data in the electronic health record (EHR).
• Perform medication reconciliation and identify discrepancies.
• Conduct telephonic outreach to coordinate preventive and ongoing care.
• Manage in-basket messages, prescription refills, and prior authorizations.
• Support referrals, scheduling, records, and DME coordination.
• Assist patients with accessing community resources.
• Prepare and maintain clinical equipment and supplies.
• Collaborate with interdisciplinary team members.
• Participate in team huddles and meetings.
• Perform clinical tasks within scope (injections, phlebotomy, POCT).
• Perform other duties as assigned.
