Qualifications

Responsibilities

• Provide exceptional primary care to adults on Medicare.
• Conduct office visits for routine and acute issues.
• Perform annual wellness visits and health risk assessments with thorough documentation.
• Care coordination with physicians, specialists, testing facilities, and care managers.
• Lead population health initiatives in coordination with the care team.
• Assist with phone triage and outreach as needed.
• Educate patients about health conditions and treatment plans.
• Participate in Oak Street Health promotional activities.
• Conduct home visits as needed.
• Other duties as assigned.
