Qualifications

  • Master of Science in Nursing, Doctor of Nursing Practice, or NP Certificate Program graduate.
  • National certification in Family Nurse Practitioner, Adult-Gerontology Primary Care NP, Adult NP, or Gerontological NP.
  • Active, non-probationary state NP license.
  • Active DEA license.
  • US Work Authorization.
  • Experience in primary care (internal medicine, geriatrics, or family medicine).
  • Understands HCC documentation, ICD-10 coding, and HRAs.
  • Passion for teamwork and cross-functional collaboration.
  • Desire to be part of an innovative population health model.
  • Bilingual proficiency in applicable areas.

Responsibilities

  • Provision of exceptional primary care.
  • Office visits for routine and acute issues.
  • Annual wellness visits and health risk assessments with thorough documentation.
  • Care coordination with other providers, specialists, testing facilities, and agencies.
  • Population health leadership in coordination with the Care Team.
  • Assisting the care team with phone triage and outreach.
  • Educating patients on health conditions, care plans, and treatments.
  • Participating in Oak Street Health promotional activities.
  • Conducting home visits as needed.
  • Other duties as assigned.
