Job summary
Oak Street Health is seeking a Nurse Practitioner to provide effective, equitable value-based primary care to adults enrolled in Medicare. You will work within an outpatient, collaborative care team including physicians, nurse practitioners, and PAs, focusing on care coordination and population health.
The role requires NP licensure, national certification, and DEA license, with full-time schedule and 40 hours per week.
Qualifications
- Master of Science in Nursing, Doctor of Nursing Practice, or NP Certificate Program graduate.
- National certification in Family Nurse Practitioner, Adult-Gerontology Primary Care NP, Adult NP, or Gerontological NP.
- Active, non-probationary state NP license.
- Active DEA license.
- US Work Authorization (visa status OK).
Responsibilities
- Provision of exceptional primary care.
- Conduct office visits for routine and acute issues.
- Administer Annual wellness visits and health risk assessments.
- Care coordination with providers, specialists, testing facilities, and agencies.
- Population health leadership in coordination with the Care Team.
