Job summary

U.S. Renal Care in Texas is seeking a Value-Based Care Nurse Case Manager to support chronic kidney disease patients through education, care coordination, and comprehensive assistance with medical history, medications, and lifestyle changes.

This role may involve telephonic or in-person visits at USRC Dialysis Clinics or physician offices. The successful candidate will collaborate with nephrologists and the dialysis team, drive key patient outcomes metrics, and be based full-time in Texas with

Qualifications

• Current RN license in Texas (and other states per contracts).
• Three (3) plus years of nursing experience preferred.
• Experience in case management and/or providing nursing care to dialysis patients preferred.
• English language communication; Spanish/bi-lingual a plus.
• Basic computer skills; proficiency in USRC clinical applications within 90 days.

Responsibilities

• Coordinate and evaluate care across the patient’s course of treatment.
• Provide health education, coaching, and treatment decision support for patients.
• Collaborate with patient, families, and healthcare providers to address needs.
• Build partnerships with nephrologists and dialysis team members to manage care.
• Drive metrics to improve patient outcomes and experiences; reduce hospitalizations.
• May conduct outbound calls and in-person visits to assess health status and adjust plans.
