Job summary

U.S. Renal Care is seeking a Value-Based Care Nurse Case Manager in Texas to coordinate care for chronic kidney disease patients.

You will work with patients, families, and the care team to educate, monitor, and adjust treatment plans, aiming to reduce hospitalizations and improve dialysis-related outcomes. The role requires an active RN license in Texas and 3+ years of nursing, with case management or dialysis experience preferred.

Qualifications

• Current RN license in Texas, and additional states as contracts require.
• Three (3) plus years of nursing experience preferred.
• Previous experience in case management and/or providing nursing care to patients on maintenance dialysis preferred.
• Demonstrated working knowledge of the English language and ability to communicate verbally and in writing, Spanish/Bi-lingual a plus.
• Must have basic computer skills; proficiency in USRC clinical applications required within 90 days of hire.

Responsibilities

• Assess, implement, coordinate, monitor, and evaluate care.
• Provide health education, coaching and treatment decision support for patients.
• Collaborate with patient, caregivers, and other healthcare providers to address patient needs.
• Build and manage partnerships with nephrologists and dialysis team members to manage care.
• Drive metrics that improve patient outcomes and experience, including reducing hospitalizations and readmissions and improving dialysis starts.
• Conduct outbound calls to assess health status and identify gaps in treatment plans.
• Lead modality patient education to support dialysis start milestones.
