Job summary

U.S. Renal Care seeks a Value-Based Care Nurse Case Manager to support chronic kidney disease patients and families, coordinating education, medical history review, medication assistance, and lifestyle changes across dialysis and transplant pathways.

Roles may be telephonic or in-person in USRC Dialysis Clinics and partner offices. You will collaborate with nephrologists, dialysis teams, and caregivers to drive metrics for improved outcomes and reduced hospitalizations; requires Texas RN license

Qualifications

Current RN license in Texas and ability to obtain/maintain licensure as required.
Three or more years of nursing experience preferred.
Prior experience in case management and/or dialysis patient care preferred.
Fluent in English with ability to communicate verbally and in writing; Spanish is a plus (bilingual).
Basic computer skills and ability to use USRC clinical applications within 90 days of hire.
Must meet state practice requirements and any background standards as applicable.

Responsibilities

Manage case management activities including assessing, coordinating, monitoring, and evaluating care.
Provide health education, coaching, and treatment decision support for patients.
Collaborate with patients, caregivers and other providers to address needs and improve outcomes.
Build partnerships with nephrologists and dialysis team members to coordinate care.
Drive metrics to reduce hospitalizations and improve dialysis starts and patient experience.
May conduct outbound calls or in-person visits to assess health status and update care plans.
