US Renal Care is seeking a Value Based Care Nurse Practitioner in Laredo to provide patient-centered clinical care and care management for individuals with kidney disease. You will collaborate with nephrologists, dialysis teams, and other providers to improve outcomes and manage chronic conditions.

Qualifications

• Current Advanced Practitioner license in Texas.
• Three or more years of experience preferred.
• Spanish/Bi-lingual a plus.

Responsibilities

• Perform physical examinations and diagnose illnesses.
• Order and assess diagnostic tests.
• Prescribe medications and manage treatment plans.
• Educate patients and families on care and prevention.
• Collaborate with nephrologists and dialysis teams.
• Maintain EHR documentation and care coordination.
