Job summary

Diverge Health is seeking a Practice Liaison to join our Market Operations team. You will be the primary contact for partner practices, coordinating with Network Development and Local Care teams to ensure smooth adoption of Diverge Health solutions and ongoing workflow improvements.

You will train clinicians and staff on value-based care, monitor performance metrics, and drive initiatives to improve quality, efficiency, and patient engagement.

Qualifications

• 2–4 years in healthcare operations or customer-facing roles
• Familiar with primary care practice workflows (scheduling, billing, documentation)
• Knowledge of value-based care (VBC) models
• Familiarity with HEDIS quality measures and utilization
• Ability to interpret reports and dashboards to track performance

Responsibilities

• Serve as the primary liaison between Diverge Health and partnered practices.
• Collaborate with Network Development and Local Care teams to establish and maintain relationships with practices.
• Support practice onboarding, trainings, and access to Diverge Health tech solutions.
• Provide education on value-based care, Diverge Health incentives, and quality/risk adjustment best practices.
• Present performance scorecards, claims data, and gap reports to translate insights into actions.
• Develop and drive value-based transformation initiatives with practice leadership.
• Assist practices in accessing Diverge Health resources and resolving operational issues.
• Maintain accurate practice profiles in the CRM and manage communications cadence.
