Job summary

Diverge Health in the United States is seeking a Practice Liaison to join our market operations team. You will be the primary contact between Diverge Health and our partnered practices, supporting onboarding and ongoing communication with the care team.

You will drive value-based care initiatives, training on our technology, and collaboration with networks to close care gaps and improve quality. Travel within the community is required and a strong background in healthcare operations is essential.

Qualifications

• Bachelor’s degree or equivalent in business, healthcare administration or related field.
• Graduate degree preferred in business, healthcare policy or related field.
• 2–4 years in healthcare operations or customer-facing role with primary care workflow knowledge.
• Experience with value-based care models and supporting practices in VBC environments.
• Familiarity with HEDIS measures and risk adjustment documentation.
• Ability to interpret reports and dashboards for performance improvement.
• Proficiency with MS Office, including PowerPoint and Excel.

Responsibilities

• Serve as primary contact between Diverge Health and partnered practices.
• Lead onboarding plans, trainings, and access to Diverge Health technology.
• Educate clinicians on value-based care and Diverge’s incentive plans.
• Present performance scorecards, claims data, and gap reports with clear actions.
• Collaborate with practice leadership to drive value-based transformation.
• Navigate EMRs and Diverge’s Provider Portal; troubleshoot issues.
• Maintain practice profiles in the CRM and manage engagement cadence.
• Support quarterly JOC meetings to review incentive performance and quality.
