Diverge Health is seeking a Practice Liaison to join our market operations team in a full-time role. You will act as the primary contact between Diverge Health and partner practices, coordinating onboarding, trainings, and access to our technology solutions.

You will educate clinicians on value-based care and incentive plans, and translate performance data into actionable improvement steps. The role requires strong communication, presentation skills, and the ability to manage multiple projects

Formación

• Bachelor’s degree in business, healthcare administration, or related field.
• 2–4 years in healthcare operations or customer-facing role.
• Experience with value-based care models (VBC) and HEDIS measures.
• Experience navigating EMRs and provider portals.

Responsabilidades

• Serve as primary contact between Diverge Health and partner practices.
• Collaborate with Network Development and Local Care teams to build relationships.
• Manage onboarding plans, trainings, and access to Diverge Health tech.
• Provide education on value-based care, incentive plans, and quality/risk adjustment.
• Present and discuss performance scorecards and data insights.
