Job summary

Diverge Health is seeking a Practice Liaison to support market operations and ensure practices understand and utilize Diverge Health’s care solutions. You will partner with Network Development and Local Care teams to establish relationships and drive onboarding, training, and ongoing collaboration across care teams.

The role emphasizes communication with providers, payers, and non-clinical staff, with travel within communities to support implementation and optimization of care workflows.

Qualifications

• Healthcare operations experience in provider or payer settings.
• Familiarity with value-based care models and HEDIS measures.
• Strong communication, presentation, and training abilities.

Responsibilities

• Serve as primary liaison between Diverge Health and partnered practices.
• Lead onboarding, trainings, and access to Diverge Health tools.
• Educate clinicians on value-based care and incentive plans.
• Present performance scorecards and translate data into actions.
• Travel to community practices to support implementations.
