Job summary

Mass General Brigham Community Physicians, Inc. seeks a Clinical Pharmacist to optimize health outcomes within our care management teams, focusing on comprehensive medication management for high-risk patients and transitions of care.

You will collaborate on ACO quality initiatives, document encounters in Epic, and support performance-based programs across payer contracts within a hybrid work model (remote with occasional assembly row meetings in Somerville, MA).

Qualifications

• Doctor of Pharmacy (PharmD) degree from an accredited college or university.
• Licensure to practice in Massachusetts (or eligible to obtain).
• Board certification (BCPS, BCACP, or BCGP) is highly recommended.

Responsibilities

• Collaborate with Care Management to deliver coordinated pharmacist support to a high-risk patient population.
• Counsel patients and perform comprehensive medication management reviews.
• Identify opportunities to improve medication adherence and outcomes.
• Document encounters and update medication lists in Epic and other EHRs.
• Support pharmacy-led quality improvement projects and payer contract quality measures.
