Job summary

Mass General Brigham Community Physicians, Inc. seeks a Clinical Pharmacist to optimize health outcomes for high-risk patients through comprehensive medication management within our care management teams.

You will conduct pharmaceutical care, perform medication reconciliation during transitions of care, and contribute to quality initiatives under our ACO contracts, using Epic and other EMR systems to document patient encounters.

Qualifications

• Doctor of Pharmacy (PharmD) degree from an accredited institution.
• Licensed to practice in MA (and NH/ME/RI eligibility).
• Minimum of three years of clinical practice.

Responsibilities

• Collaborate with Care Management teams to support high-risk patients.
• Provide direct patient care, counseling, and medication management.
• Perform comprehensive medication reviews and transitions of care reconciliation.
• Support ACO quality metrics and performance initiatives.
• Document encounters in Epic and other EHRs.
