Mass General Brigham Community Physicians, Inc. seeks a Clinical Pharmacist to optimize health outcomes within our care management teams, focusing on comprehensive medication management for high-risk patients and transitions of care.
You will collaborate on ACO quality initiatives, document encounters in Epic, and support performance-based programs across payer contracts within a hybrid work model (remote with occasional assembly row meetings in Somerville, MA).

The Clinical Pharmacist optimizes health outcomes by providing the highest level of patient care for a high-risk patient population within our health system. In this role, the Clinical Pharmacist applies subject matter expertise to manage the pharmacy component of high-risk patient care management. As a member of Care Management teams, this role focuses on comprehensive medication management reviews, therapeutic alternatives, medication adherence quality performance, and post-discharge medication reconciliation. The individual will provide primary care-focused pharmacotherapy services to evaluate and manage patients with complex chronic primary care conditions (e.g., CKD/ESRD, Type 2 diabetes, hyperlipidemia, hypertension, COPD/Asthma) through direct patient care, as appropriate. Additionally, the Clinical Pharmacist may align clinical pharmacy workflows to support and manage quality in our ACO contracts, ensuring the health system achieves key performance benchmarks and success across various performance-based initiatives.
Licensure & Education:
Degree: Graduate of an accredited college or university with a Doctor of Pharmacy (PharmD) degree.
Licensure: Currently licensed or willing to get licensed to practice pharmacy in the Commonwealth of Massachusetts, NH, ME and RI.
Certifications: Board certification (BCPS, BCACP, or BCGP) is highly recommended.
Experience: Minimum of three (3) years of related clinical practice experience.
Preferred experience functioning as a clinical pharmacist within an ambulatory care, health system population health department, or managed care setting.
Experience executing performance-based initiatives and tracking clinical outcomes to satisfy payer or ACO contract quality measures is preferred.
Additional Knowledge, Skills & Abilities:
Communication & Collaboration: Excellent verbal and written communication skills; outstanding interpersonal skills with a proven ability to work collaboratively in a multidisciplinary team.
Healthcare Knowledge: Deep understanding of healthcare delivery systems, community resources, and managing clinical quality and adherence metrics within ACO contract frameworks.
Core Competencies: Must be self-directed, proactive, organized, and possess strong time management skills; ability to work efficiently in a high-intensity, fast-paced environment both independently and as a team player.
Technical Skills: Proficient in Microsoft Office (Word, Excel, PowerPoint) and experienced with electronic health records (Epic preference).
Flexibility: Ability to be flexible with working hours and adapt to a dynamic, hybrid workflow
Schedule and Work Model: Full-time, Monday through Friday, standard business hours.
Hybrid Work Model: Primarily remote / work from home but must be flexible to attend meetings at Assembly Row in Somerville, MA as needed. On remote workdays, employees must use a stable, secure, and compliant workstation in a quiet environment. Teams Video is required and must be accessed using MGB-provided equipment.
