Job summary

Qualifications

• PharmD degree required; licensure in Massachusetts or eligible.
• BCPS/BCACP/BCGP certification highly recommended.
• Minimum of three years of related clinical practice experience.

Responsibilities

• Collaborate with Care Management teams to provide pharmacist support for high-risk patients via phone, telehealth, or visits.
• Provide patient counseling using motivational interviewing to improve health literacy and adherence.
• Document encounters, update medication lists, and communicate with providers via Epic and other records.
