Job summary

This full‑time, hybrid role requires a PharmD, Massachusetts licensure, and BCPS/BCACP/BCGP certification is recommended.
Qualifications
- PharmD degree from an accredited college or university.
- Massachusetts license to practice pharmacy (NH/ME/RI optional).
- Board certification (BCPS/BCACP/BCGP) recommended.
Responsibilities
- Collaborate with Care Management teams to provide pharmacist support.
- Counsel patients and promote adherence using motivational interviewing.
- Perform comprehensive medication management reviews for high‑risk patients.
- Document encounters and coordinate with providers in Epic.
