Job summary

Premier Community HealthCare Group seeks a strategic Manager, Value-Based Care to lead value-based performance across managed-care and ACO portfolios. You will align clinical operations, risk coding, data analytics, revenue cycle, and leadership to improve outcomes and contract performance.

You will own VBC metrics, drive data-driven action, close care gaps, and support Premier in reaching contract targets while advancing value-based care across the organization.

Qualifications

• Bachelor’s degree or 10+ years of relevant experience as determined by HR.
• High School diploma with commitment to obtain a degree within a reasonable period may be considered under PCHG guidelines.
• Five years of healthcare experience, including two years in value-based care, population health, quality improvement, or risk adjustment.
• Experience leading teams or projects.
• Strong understanding of value-based care, population health, quality measures, care gaps, and performance metrics.
• Ability to analyze data, identify opportunities, and develop actionable strategies.

Responsibilities

• Lead Premier’s VBC program and partner with Senior Leadership on strategy, growth, and sustainability.
• Drive provider engagement, risk-score capture, coding accuracy, and resolution of documentation and continuity-of-care gaps.
• Own VBC dashboards, payer scorecards, ACO data, risk scores, seen rates, and care-gap performance.
• Lead cross-functional gap-closure and coding huddles and prioritize the highest-value opportunities.
• Oversee AWV, newly attributed-member, patient outreach, and redetermination-retention workflows.
• Partner with Quality and Marketing to develop campaigns that improve care-gap closure and patient-seen rates.
• Optimize Epic alerts, registries, and supplemental-data workflows so delivered care is accurately captured and credited.
