Job summary

Premier Community HealthCare Group seeks a strategic Manager, Value-Based Care to lead VBC performance across managed-care and ACO portfolios. You will align clinical operations, risk coding, data analytics, revenue cycle, and senior leadership to improve outcomes and contract performance.

This role reports to the leadership team as a VBC subject-matter expert and drives value-based care across the organization.

Qualifications

• Bachelor’s degree or 10+ years of relevant experience as per HR guidelines.
• Five years of healthcare experience, incl. value-based care, population health, quality improvement, or risk adjustment.
• Experience leading teams or projects.
• Strong understanding of value-based care, population health, quality measures, care gaps, and performance metrics.
• Ability to analyze data, identify opportunities, and develop actionable strategies.

Responsibilities

• Lead VBC program strategy with Senior Leadership on growth and sustainability.
• Drive provider engagement, risk-score capture, and documentation continuity gaps.
• Own VBC dashboards, payer scorecards, ACO data, risk scores, seen rates, and care-gap performance.
• Lead cross-functional gap-closure and coding huddles to prioritize opportunities.
• Oversee AWV, newly attributed-member outreach, and redetermination-retention workflows.
• Partner with Quality and Marketing to improve care-gap closure and patient-seen rates.
• Optimize Epic alerts, registries, and supplemental-data workflows to ensure accurate capture and credit.
